About Semaglutide

Semaglutide is a long-acting glucagon-like peptide-1 (GLP-1) receptor agonist. FDA-approved as Ozempic (2017) for type 2 diabetes and as Wegovy (2021) for chronic weight management in adults with obesity or overweight with comorbidities. An oral formulation, Rybelsus, is also available. Extensive phase-3 trial data — the STEP and SUSTAIN programs — underpins both approvals.

With a half-life of approximately 7 days (168 hours), semaglutide is dosed once weekly subcutaneously. Typical titration starts at 0.25 mg/week and escalates every 4 weeks to a maintenance dose of 1.0–2.4 mg/week, depending on indication and response. Compounded semaglutide (from licensed 503A/503B pharmacies) has been widely available in the U.S. market during 2023–2026 amid branded-product shortages, though the FDA's 2024 declaration of resolved shortage has evolved the regulatory landscape.

What's the standard titration schedule?
FDA labeling for Wegovy specifies: 0.25 mg/week for 4 weeks, then 0.5 mg for 4 weeks, then 1.0 mg for 4 weeks, then 1.7 mg for 4 weeks, then 2.4 mg as maintenance. Titration is clinician-directed; apply the "GLP-1 Onboarding" template to pre-populate this schedule in your log.
How do I convert mg to mcg?
1 mg = 1,000 mcg. So a 0.25 mg dose = 250 mcg. Enter 250 in the "target dose" field (mcg is the working unit for the calculator).
What's a typical compounded vial strength?
Compounded semaglutide commonly ships as 2 mg, 4 mg, or 8 mg vials. Reconstitution volumes vary — check your pharmacy's instructions. The calculator accepts any vial-strength and BAC-water combination.
